Hi All,

I'm trying to locate the CMS ADL questionaire for cataract patients. All of our cataract surgeries are being pulled for pre-payment audit and in doing some research it looks like we should have this CMS ADL questionaire for cataract patients in our chart. I can't find this form anywhere. Does anyone know where this form is located on the CMS website or where I can find one? This isn't my field of speciality so I'm hoping the experts here can point me in the right direction.
