A provider documented time in the chart but his documentation can get him a higher level. Do you code the documented time or give the higher level based on his documentation?
 
As per 2021 E&M guidelines which ever leads to higher level should be considered out of time and medical decision making. So this allows the clinicians to choose the E/M visit level based on either medical decision making or time
