What codes/modifiers should be used for the following:

PROCEDURE: Bilateral peripheral angiogram with selective angiography
of the right and left legs. Angioplasty and stenting of the right
common iliac with an 8 x 59 Viabahn and angioplasty and stenting of
the left common iliac with an 8 x 39 Viabahn. Intravascular
ultrasound of the right iliac.
I have 75716-26, 37221-50, 37252-59

Hi,
I have been a cardiology coder for years. If the patient has had a CT of lower extremities, then 75716 would bundle into 37221 therefore it is not reportable. Also, you would not need a modifier 59 however you may need lateral modifier depending on the payer. Also be careful that it is two separate lesions and not one long lesion extending to each vessel.
