Wiki LTC with salpingectomy

I have an op note that reads: The right fallopian tube was cauterized with a bipolar cautery. The left tube was cauterized. The left tube stuck to the instrument and upon removal, the tube was ripped, at which time a left salpingectomy was performed.
Now, do I code this 58661, 58670? DX for the 58661 would I use a E870.0 or the V25.2?
Would appreciate help on this.
