Coding L5, S1, S2 RFA

When coding JUST the L5 nerve (not the facet joint) and S1 and S2 nerves, how would you code?

I am now using 64635 for L5 & 64640 x 2 for S1 and S2. Would 64640 x 3 be correct?

Any advice would be appreciated.

Even though I don't bill lateral branch radiofrequency (WPS Medicare J5 doesn't cover them) and the physician's I work with are not performing them. I have believe since it is for treatment of branches that innervate the SI joint, I believe that L5 portion was contributing to the SI joint in the instance for the given procedure, and potentially 64640 x 3 should be reported. Instead of 64635 since it is not treatment of Facet joint as the pain generator.
