Pt had a Lumpectomy. Should I use Dx Code Z98.890 as 'Personal Hx of Lumpectomy'?

If, per the physician's documentation, that part of the patient's history is relevant to the encounter you are coding, and you don't have any more specific information about the procedure, then yes, that would be the code to report.
