I have never run into this before so I am not sure how to go about billing for this or if there is really anything different I need to do.
We give patients Lupron injections and our supply of 3 month and 6 month are on hold. We can still get the one month which is fine but all the NDC numbers are different so I am not sure on how to bill this out on the claim form because of that. My mind is telling me to bill the one month Lupron with 3 or 6 units since that is the NDC we will be using. Can anyone help me out on this?????

J code is determinate on the drug, not the NDC. All NDCs will have the same J code. So you would bill the J code with the appropriate number of units based on the milligrams. Of course you would include the NDC actually used on the claim form. I hope this helps clear up the confusion. Let me know!
