How are you billing for the Lupus Evaluation Profile - CPT 85598 / 85730 / 85732? Can we bill for all 3 CPT's (add modifier to 85730 and 85732) or bill only for 85598 and GZ CPT 85730 and 85732 (OP, same dos and specimen)? Please advise.
