I am looking for a HCPCS code for Magnesium Sulfate billed with D5W. Since there is a shortage on just purchasing Magnesium Sulfate by itself we have been purchasing it already mixed with D5W and I cannot find a HCPCS code for premixed bag of D5W with Magnesium Sulfate. Also when you are billing the administration for Magnesium Sulfate and KCL whether it is mixed with NS or D5W and you have 96413 as your initial would you bill 96367 non-chemo sequential or 96361 additional hydration?
