Would you code a malunion repair or arthrodesis? Previous ORIF and wound dehiscence

Incised along previous scar line of distal fibula. Wound deepened to bone. Plates and screws identified. Screws removed. Malunion site was debrided. Utilizing osteotome, the syndesmosis ligament was taken down. The malunion and shortening of the distal fibula was restored and temporary stabilized with K-wire. Next, fixation was undertaken with a locking plate and screws across the proximal distal fibula. Defect of 1.5 cm was bone grafted. The syndesmotic screw was placed for better fixation. There was a persistent joint line consistent with an incompetent ankle ligament. The ATFL and CFL were repaired. Bone graft was packed into the malunion site. Gastrocnemius recession was then performed.

You have to read what the specific procedure in the documentation. You can't code if the Operative Report doesn't state the name of the actual procedure. This wouldn't be code. You would have to query the surgeon to clarify what surgery he did on the patient. Giving a description doesn't validate the procedure. In the operative report should state the name of the procedure and other information.


 
Yes, it states the procedure-repair of malunion w/bone graft . I wouldn't code solely from the heading of the report, as this may not always be correct for the procedure that is performed.
