Can a massage therapist bill a 97140 under a Physical Therapist? Thank you for any help you can give me. Trish
 
How about under a physician? We have athletic trainer lmt that works in our office. How would I bill 97140 under the physician? What does that look like on a claim form and does the physician have to be in the office that day in order to bill correctly?
