In Alaska it is my understanding that they can only bill insurance under the direct supervision of a chiropractor or physician. My problem is I have come across several clinics in my time that when billing the the massage they bill the rendering provider as the the physician or chiropractor. Is that correct? Or should the rendering provider be the the LMT and then the chiropractor and physician be put in box 17 as ordering/supervising provider on the HCFA . I can't seem to find any answers anywhere that's correct for Alaska.
