Wiki massage therapy coding, Indiana??

What is the proper format for billing/coding a massage therapist's services if they are practicing in a chiropractor's office within the state of Indiana? I believe the correct CPT would be 97124 but does the therapist use the chiropractors NPI, or does the state of Indiana allow billing by the massage therapist individually?
