I need some help. I have a patient that was not known to our office. She was in a fatal car accident and brought to the ER and pronounced deceased. Her estimated gestational age was 30 weeks. No history on the patient. A stat c-section was performed in the Emergency department to try to save the infant. (Infant heartbeat could not be found) . Infant was not able to be resuscitated by Neonatology and was also pronounced dead. I would assume C-section only for the CPT (59514). We are billing for the physicians services only, not the facility. I am having trouble with the ICD10 code for the maternal death- Nothing I have found seems to be appropriate. Thanks for any advice

Do you have access to what the ED provider states as the final diagnosis? If not, and you are coding without information, I would look at the following codes for the mother's chart: O9A22 ( Injury, poisoning and certain other consequences of external causes complicating childbirth), T40.90xa ( Injury, unspecified, initial encounter) as we don't know the specific injury of the mother aside from her passing away. V892XXA (V89.2XXA: Person injured in unspecified motor-vehicle accident, traffic, initial encounter) and maybe Z37.1 for the outcome of the delivery of the infant not making it. This was the best I could come up with based on the limited information provided. I hope this helps. Best of luck, and my heart goes out to the families of everyone involved.
