Wiki MCD billing question

This is how we are billing a destruction by neurolytic agent/radiofrequency:
64633 RT
64634 RT
64634 RT 59
64634 RT 59

However our A/R dept. is stating they are having to file corrected claims with MCD to show them billed like this.
64633 RT
64634 RT
64634 RT 59 x2 units

That seems odd to be. Which is correct and why?

You need to check with your carrier. This may be their policy. Some carriers want individual line items and some want one line item with # of units provided. Usually you can find this information on the carrier website or you may have to call them.
