Wiki MD sees husband and wife at same time?

We've got a provider who saw a husband and wife at the same time. The entire visit is going to be coded based on time. He spent an entire 75 minutes on counseling and coordination of care. I don't see any official rules for this. Has anyone encountered this? I'm trying to figure out if it would be appropriate to bill a level 5 for EACH patient.
 
I would say that in order to bill based on time, the physician would need to speak to one at a time. Not both at the same time. And then have the start and stop time for both and the counseling time for both. You can't be couseling two people at the same time. It just does not make sense any way you look at it.
