If we see a patient that had commercial insurance and then they come back in for a consult for a new problem and was referred by another physician and now they have MDCR, am I supposed to code established code or since this is the first time we have saw them with having MDCR can I bill new patient code? Hope that makes sense. Thanks in advance for any feedback.

If the return visit is within 3 years of the last visit it is an established patient regardless of the insurance or that the patient changed carriers.
