Wiki MDM and diagnoses

If a patient presents to your practice as a new patient and has diagnoses in their history ex: diagnosis of breast cancer in 2011 and presents to your oncology practice as a new patient when calculated the MDM do you count that as a new diagnosis because the patient is new to you or a chronic illness/problem?

No I would use a past history dx code for the breast cancer. Check the dx blocks of Z85, Z86 or Z87. I hope this helps.
