For column 2, Category 3 (data section) when discussing management or test interpretation with an external physician, must the physician be named so that we know its not a physician in the same practice?
 
Hi there, I haven't seen a specific guideline or policy to that effect. But complete documentation would include the names of people the treating provider discussed the patient's care with. It also proves that the discussion took place at all.

Keep in mind that it can be a provider from the same practice if they're different specialties.
