If the provider bill for depression and anxiety with different dx codes. Is this to be counted as 1 or separate problems addressed for the MDM?

Hi there, it is based on the problems the provider actually addressed at that encounter, not the number of diagnoses listed. If the documentation shows they addressed the depression and the anxiety you could count both.
