I'm auditing a record that documents a skin lesion (probable carcinoma) as the diagnosis, and the physician is recommending excision of the lesion. Would credit be given for Low Risk (Minor surgery with no identified risk factors) or for Moderate Risk (Undiagnosed new problem with uncertain prognosis)?

I also agree with moderate, based on the potential severity of the problem itself (and not necessarily the risk associated with the treatment, although it doesn't hurt).
