I got the answer for my question but still iam in confusion, History and Exam comprehensive and MDM is Moderate so for new Pt we need 3 out of 3 components so we code 99204 based on MDM.

For est. pt we need only 2 components out of 3, so if we look into the above scenario the history and exam are qualified for level 99215, but my question is should we code based on the MDM or the EM guidelines which states only 2 out of 3 are needed for est.pt though the MDM is moderate, i want someone who wrote CEMC and passed to answer because iam preparing to write CEMC EXAM and this will be a great help to me.
