Patient is offered surgery or stick with PT. Patient declines the surgery and decides to stay with the PT. Would you give "moderate" level of risk per the risk table since surgery was a consideration but the patient declined it? Or would you give "low" because that is what was chosen?
 
MDM is the medical decision making of the doctor. If he is medically deciding to do surgery, I would give him credit for that decision whether the patient decides to have the surgery or not. So, yes, I would give him moderate risk (if it is for major surgery) for his decision to do surgery. :)
