The physician documented 4x4 cm for for benign lesion of the neck.
Would the correct CPT code to report be 11424 or 11426?

That would depend on if the 4cm is the lesion size or the excised diameter (lesion diameter plus narrowest margin). It sounds like that is the lesion size so the total excision would be over 4cm and you would use the 11426.
