Medi-cal does not cover any physical therapy evaluation codes. ... Do you have any advise how to get this claims paid?

All physical therapy services must be pre-authorized.

In Box 19, the words "Initial Evaluation Visit" or "Six-Month Re-Evaluation visit" must be shown.
