Pt comes in to be evaluated for mmt. She had a tear back in 2012 that she wasn't able to have sx for due to other reasons. She is coming in now in 2014 and doctor has dictated mmt. Would you code this as a current injury or old injury? Thank you (836.0 vs 717.0)
 
It is an old condition. Was there an actual injury or just degenerative changes? Or degenerative conditions exacerbated/aggravated by activities of daily living? Based on the current documentation, you should be able to determine if she is having an acute flareup of the old condition, or is asymptomatic, or whatever the doctor documents.
 
This is exactly a document deficiency and the physician should be queried whether this is an acute tear or flare up of old tear. As the patient didn't undergo Sx for the tear, it would likely be 717.3.
