I am working on Gastroenterology and am receiving huge claim denials as Medicaid ms being secondary and they always deny due to Member is inelligibe for this services, Even though the CPTs are of E&M, I reached to them and they said its issue with your CH, But i also submitted some claims from Medicaid portal but again got the same denial, Does anyone has faced that issue and how to resolve that.
