In this scenario E11.22 only or E11.22 and N18.9 to code?

In the code E11.22 mention use additional code n18.1 to n18.6, But in the code N18.9 Code first E11.22 is there.

Hi,

I am currently looking in 2019 ICD-10 Coding and have a question for you. Does the documentation by the provider state the patient has hypertension? The reason I'm asking is to code E1122 indicates to direct the coder to code N18 as well but lists N181-N186, not N189. If the patient has HTN then this relationship can be assumed (unless provider states HTN unrelated) and coded as I129-HTN CKD Stage 1-4 or unsp where you could code N189. Otherwise, query the provider for the stage of CKD because E1122 seems to need the stage of CKD to properly assign the N18 series code.
