We have some disagreement within the office on how to give points for Decision Making. Example: One of our Doctors sees a patient in the hospital for a new onset of a-fib. Patient had a cardioversion for a-fib in 2009. Then we seen the patient in our office as a new patient in 2010 for post-op a-fib. When the patient is seen in the hospital for a new onset of a-fib would you consider this a new problem with additional work-up (4) points or established problem worsening (2) points. Thanks for your input.
