If new pt presents w/severe knee pain, xrays ordered & showed bone-on-bone adv. osteo so TKA scheduled would it be a 99204 or 99205. Pt. has no underlying health issues. Presenting problem would be high but no identified risk would make mgmt options moderate...

Does well controlled high blood pressure count as risk factor...same for oral medication controlled diabetes?
