Has anybody else been gotten any denials for medical necessity for the 78452 by Medicare in northern California? All of a sudden every time I bill this code it is being denied and it doesn't matter what diagnosis i use. I have checked the bulletins and the alerts on the Noridian website and i can find anything. We do a lot of these and this just started happening a few months ago.
