medical necessity for J0610 with Oxaliplatin

I thought using 357.6 neuropathy secondary to a drug would cover the medical necessity for J0610 calcium gluconate used with Oxaliplatin, but my "scrubber" keeps saying it isn't meeting the standard... charged as 96367 and J0610... would appreciate your input :)
 
You need to check and see if the patient's insurance carrier has an LCD for the Oxaliplatin. Most payors limit the use of Oxaliplatin to a neoplasm of some sort that is what your scrubber is probably looking for.
