Can an auditor tell a physician from his/her documentation that a service was not medically necessary or can only a practicing medical peer tell a physician it was not necessary?
 
I think you can do this either way.
If I as a coder/auditor question a medical records necessity I will question it. We are pretty savy to medical necessity and what is ar is not right, if something does not appear right it probably isn't. You can also ask for a physician peer review. The providing physician whos document is in question may be more preceptive to hear an opinion from another physician.
