Can anyone tell me if I can bill both medical and vision on a single office visit?
 
I believe that you can only if there are two different specialties involved (ie. optometrist sees the patient for routine vision care and refers them to a retinal specialist for a detached retina.)
 
Ophthalmology uses both e/m codes & vision codes. You can only use either e/m code or vision code to bill. you can bill refraction (92015) w/ either set of codes if it's performed. Usually optometrist use the vision codes because these are widely used w/ vision insurances. Ophthalmology uses both because they can bill both vision & medical insurances.
 
Our office does retinal and anterior seg photos, and I have billed those medically with a medical dx on the same claim as a routine exam with a routine dx. The key is the insurance.

I think the provider can not bill two insurances ( Medi-Cal and VSP ) for a single visit because Medi-Cal vision coverage is through VSP.
