Wiki Medicare adv no auth denial

We have an aetna medicare part c plan denying for no auth when we have a print out from availity showing no auth required. Appeals denied even with print out and medical necessity proof. Aetna provider rep ignored the proof as well. We are in network so can't go to maximus. Any next step?
