Wiki Medicare Advantage Preventive/Mammogram same day

We bill our Medicare Advantage plans with the Preventive Care Codes (99387-99397 or other age appropriate codes) we also perform a Screening Mammogram on the same day. We have never used a modifier for these services. I had one of the plans request a refund on the past 2 years of services billed with the reason code being 024 (EM VISITS NOT ALLOWED WITHOUT THE APPROPRIATE MODIFIER) is anyone else having this problem. I do not have any other plans that I am having this problem with and can not find information on the Medicare website since Preventive Care is a non covered service with them.
 
United Healthcare does this for all kinds of services. The screening mammo has a global of XXX so they want mod 25 attached to the e/m if appropriate.
