Hello All,

I was hoping somebody can help me. Medicare has taken back our payments for a perfusion for Meniere's disease and sudden hearing loss.
They state not medically necessary however there is no LCD and Medicare will not give me information on where to find a policy.
Does anybody know the policy or where I can find the policy?
