Wiki Medicare Annual Wellness visits

Hello, I am curious if any of you have any input or resources around coding for Medicare Annual Wellness visits, specifically regarding whether you just code the wellness visit as a single code for the visit or also include the patients active diagnosis' on their problem lists that have been reviewed at the visit.

Hi, As per my knowledge we code active diagnosis to the E/M if documentation supports and preventive care separately and if documentation states patient only came for routine wellness exam, ordered labs along with there's any already existing conditions which the document must state either in HPI that other acute or chronic conditions are also being reviewed in that DOS can be coded with preventive visit CPTs mostly non Medicare CPTs (Age based codes., 99381-99397). I have observed that Medicare CPT (G series codes) usually gets only tagged alone with preventive dx codes. Correct me If I'm wrong, would like to know if there's something I'm missing out on.
