Please advise how to bill cpt 11045 frequency of 40?

The MUE for the 11045 is 12 so you can only bill for 12 units. How many sq cm are being debrided? If more than 260 sq cm are being debrided I would use a -22 modifier and provide the documentation showing how much debridement was done.
