Hi All,
I received a denial from medicare with this remark code 16

I have reviewed the claim and do not see where there is an error.

Can anyone assist?

Medicare CO-16 denials are usually accompanied by an additional RARC code (coding starting with M or N, e.g. MA81 or N248) which may give you additional information about the reason for the reject/denial. If not, or if you still cannot determine what is causing the error, then you really have no choice but to contact the carrier and ask what the problem is.
 
thank you yes i found the answer, the patients name that came to our office on the facesheet is not as medicare has it on file. will be correcting.
