Does anyone have any issues with getting denials for add'l levels with Medicare? Ever since our Jurisdiction E switched from Palmetto to Noridian- it seems that we are not having good luck with getting reimbursed.
ex 63047 addl levels 63048x2... getting paid for 63047 and only one level of 63048 or solely 63047 and no payment for 63048

No I have not seen any issues with this. I would definitely suggest to check there website to see if you can find any frequency limitations/guidelines on the code. What is the denial code you are getting?
 
And they haven't previously processed the line? Usually duplicate claim/service means they already processed the line item. So there should be either another denial code or payment for that line item.
