Why is the patient fatigued? Sounds like provider needs give better documentation and definitive dx code as to reason. Is the fatigue due to Anemia, Cancer, Insomnia dx G47 Cardiac, Poor nutrition, dx E63, Muscle instability ,M25.3 DM dx E11, medication reaction, lack of vitamins dx E55 or E61 or overuse of muscles X50 dx?
