Can anyone point me in the direction of finding Medicare's guidelines for documentation of Assistant Surgeons? My physicians don't exactly believe what I'm telling them needs to be documented when an assistant is used during surgery. They want something in black and white and of course I'm having a difficult time finding what I need on the CMS and Palmetto GBA websites.
