WHAT IS THE PROPER WAY TO BILL 4 EKG'S THAT WERE DONE ON THE SAME DOS BY THE SAME PROVIDER?
ARE THEY BILLED ON 1 LINE WITH 4 UNITS?
OR ON SEPARATE LINES WITH THE 76 MODIFIER ON LINES 2-4?

I agree with heartyoga. According to Novita's website they specifically say to bill it out on separate lines with mod 76 on the subsequent lines.
But you may want to check you MAC to see what they say.

"Bill all services performed on one day on the same claim.
Report each service on a separate line, using a quantity of one and append 76 to the subsequent procedures.
Documentation must support the use of the modifier."
