Wiki Medicare excluded providers?

Is there information anywhere of what can get a provider excluded from Medicare? We don't see many Medicare patients but want to make sure we doing everything correctly. Our manager thinks she heard there was an updated list of things Medicare looks for that came out in January. I can't seem to find anything online, except an excluded providers database, but hat doesn't help. Any help is appreciated.

Exclusions are reserved for serious offenses including criminal convictions, fraud or loss of license, but default on a student loan is also listed as one of the categories. The OIG web site has more information about this - here's a list of the categories of exclusions:
