Has anyone heard about any changes in Medicare guidelines for billing Hep B and adm? I received a few denials from Medicare for this type of service. Denial code N265 Missing/incomplete/invalid ordering provider primary identifier.
 
Sounds like it has something to do with your provider's profile. Is the provider new or associated with another NPI?
