Help! Either my physician's software is not properly updated or I missed a big change in Medicare formatting or modifier usage.

Ophthalmologist's diagnostic testing such as visual field 92083 and Ascans being denied and I am not sure why. I am trying to research... Is it the new 02/2012 form and I am or software is not answering a question(s) properly, or is a modifier I should be using all of a sudden? Please help. Last time I had a question on this site not one person answered.. I don't know why. But I am hoping someone will help this time. :)
