Wiki Medicare Op reports

We have a pain management provider whose claims are always denied for op reports. I started to send them to Medicare via paper with the report to the PO Box 7111 address and the claims are now denied because they cannot be sent via paper. HELP!

you cannot send anything on paper to Medicare......
you must send your claim electronically as always, but, in box 19 you must type the word
"FAX". Then within 48 hours you need to fax the appropriate report for that claim with the Medical record cover sheet that Medicare provides. Look on your MAC's website for this form as each MAC has their own. You fill out the form, attach the necessary records for that claim in question and fax it all to the number listed on the form that your MAC provides.
