I am billing for Critical Access Hospitals and we see incredibly too many rejections that state the following:
A7:228: The claim has invalid information as specified in the status details and has been rejected.; Type of bill for UB claim
First of all, what is this? Second of all, short of handkeying directly into the Medicare DDE what can we do?
Also, yes they are legitimate inpatient stays and everything else looks to be correct.

Help!

There's no way to tell what exactly is causing this without looking more carefully at the claims, but I'd suggest getting on the phone with both your payer and your billing system's support people. This doesn't sound to me like a rejection that is due to an error in coding or billing, but more like a mapping problem in the programming of the billing system that may be causing an invalid code to be sent through to your payer.
