Wiki Medicare Revoked Billing Privileges Reconsideration

Hi,

A provider I recently started working for has a lapse in billing privileges for Medicare (about a week) due to a late revalidation application. He is an individual provider with a private practice and there were some circumstances that led to no management for a while, which led to the late revalidation (he himself did not know he had to revalidate or how to since it was taken care of by the previous manager). He did see some patients in office and in hospital, so what can I write to get this reconsidered and allow him to get paid for that week long lapse? I have the form and a letter draft ready, but any input would be nice on is accepted and what is not.
